How do you know when a client is making meaningful progress in individual therapy?

Caitlin Opland
Caitlin Opland

Licensed Clinical Social Worker (LCSW)

I know a client is making meaningful progress in individual therapy when they demonstrate a deeper understanding of their thoughts, emotions, and behaviors, coupled with increased self-awareness and insight. This often manifests in improved coping skills and emotion regulation strategies, allowing them to manage stressors more effectively. Reductions in symptoms associated with their presenting concerns, such as anxiety or depression, further indicate progress. Additionally, clients may exhibit healthier communication patterns and boundaries in their relationships, reflecting positive changes in social interactions. As therapy progresses, clients become more engaged and motivated, actively participating in goal-setting and taking ownership of their therapeutic journey. They begin to apply therapeutic skills and insights to real-life situations, leading to a sense of empowerment and agency. Ultimately, clients experience an enhanced overall well-being and quality of life, expressing greater satisfaction and fulfillment in various aspects of their lives.

What should someone do to prepare for starting depression counseling?

Caitlin Opland
Caitlin Opland

Licensed Clinical Social Worker (LCSW)

If someone is hesitant to come to their first depression counseling session, I would tell them that it is completely common. That our job as therapists is to make you feel safe and welcome to share whatever you feel comfortable telling in that space the first time. Know that we will never judge you for how you feel or what you want to share. It is very common for people to fear judgment, but our role is to only encourage your healing journey and offer a safe place for perspective if needed.

What is your go-to approach for anxiety therapy?

Caitlin Opland
Caitlin Opland

Licensed Clinical Social Worker (LCSW)

I have a few approaches that I like to use with my clients and their anxiety symptoms. I generally use cognitive behavioral therapy and solution-focused therapy approaches. These help the clients see their maladaptive behaviors and thought patterns and allow them to more quickly and easily find solutions to move forward. I find that clients appreciate this approach the most if they are asking, “How do I fix this?” more so than, “Why do I do this?”

What tools do you teach in cognitive behavioral therapy?

Caitlin Opland
Caitlin Opland

Licensed Clinical Social Worker (LCSW)

Central to my approach is cognitive restructuring, where I help clients identify and challenge negative or irrational thoughts that fuel their emotional distress. By examining the evidence for and against these thoughts, clients learn to reframe their thinking and develop more adaptive beliefs about themselves and their experiences. Additionally, I incorporate behavioral activation techniques to encourage clients to engage in pleasurable or meaningful activities, thereby alleviating symptoms of depression and increasing overall well-being.Exposure therapy is another critical component of my approach, particularly for clients struggling with anxiety disorders. Through gradual exposure to feared or avoided situations, thoughts, or memories, clients learn to confront their fears and develop coping strategies to manage their anxiety more effectively. This may involve systematic desensitization, imaginal exposure, or in vivo exposure, depending on the nature of the client's anxiety. In addition to these techniques, I provide clients with psychoeducation and practical skills to better cope with stress, manage emotions, and improve interpersonal relationships. By teaching relaxation techniques, communication skills, assertiveness training, and problem-solving skills, clients gain the tools they need to navigate life's challenges more effectively. I assign homework between sessions throughout the therapeutic process to reinforce learning, practice new skills, and track progress. By actively engaging in homework assignments, clients can apply what they've learned in therapy to real-life situations and accelerate their progress toward their treatment goals.Regular monitoring and feedback are also essential aspects of my approach to CBT. By closely monitoring clients' progress and regularly soliciting feedback, I can assess the effectiveness of interventions and make adjustments as needed. This ongoing dialogue ensures that clients feel heard, supported, and empowered to take an active role in their treatment journey.
